Empowering clinical pharmacists to manage heart failure and hypertension effectively and improve outcomes for patients

Our interactive workshops will combine expert speakers and sharing of best practice by our clinical pharmacist facilitators who are conducting Heart Failure and Hypertension related services in their own practices.

The Heart Failure & Hypertension i2i program is fully funded by Novartis Pharmaceuticals UK Limited.  All content has been developed by Soar Beyond with input from the expert faculty.

Speakers

Rocco Hadland

Rocco works as a Primary Care Network clinical pharmacist for South Wirral PCN as well as being part of the GP clinical pharmacist integrated team between Wirral University Teaching Hospital and GP practices across the Wirral. He is a member of the UK Clinical Pharmacy Association (UKCPA) heart failure committee, and a member of the editorial advisory board for the Clinical Pharmacist journal.

Dr Rosie Heath

Rosie is a CVD GPwSI and has worked closely with practice pharmacists to develop chronic disease management clinics as well as setting up a HCA led hypertension clinic. She is also an observer on the board of the British Society for Heart Failure.

The workshop will focus on ‘insight’ in the morning and on ‘implementation’ the afternoon. The day has been structured to allow plenty of networking time and activities
