Our interactive workshops will combine expert speakers and sharing of best practice by our clinical pharmacist facilitators who are conducting Heart Failure and Hypertension related services in their own practices.
The Heart Failure & Hypertension i2i program is fully funded by Novartis Pharmaceuticals UK Limited. All content has been developed by Soar Beyond with input from the expert faculty.

Alison has worked within cardiology for over 20 years leading the pharmacy team managing cardiac patients and has developed the role as a pharmacist prescriber running medication optimisation clinics for patients with heart failure in primary and secondary care.Read Bio
Jackie has been a Nurse for more than 18 years and a Heart Failure Nurse Specialist for 11. She spent some time in Vascular Surgery and General Medicine before moving to Cardiology and gained an interest in Heart Failure.Read Bio
Jaggy is a passionate remote practice based prescribing pharmacist at GP at Hand, the NHS arm of Babylon Health. Jaggy is also the anticoagulation service specialist pharmacist for Bristol Myers Squibb. With a career spanning 21 years across almost all sectors of pharmacy, Jaggy has a proven track record of joint working in designing and implementing remote and patient facing services and quality improvement initiatives.
With a specialist interest in cardiovascular medicine, Jaggy has extensive experience in anticoagulation, hypertension clinics and medication reviews. Jaggy is the clinical lead for Wessex Network of Evolving Practice Pharmacists (NEPP) forum, a regional hub to support practice pharmacists in long term condition management. In addition, he has considerable experience in setting up pharmacists led long term condition management clinics in practice.Read Bio
The workshop will focus on ‘insight’ in the morning and on ‘implementation’ the afternoon. The day has been structured to allow plenty of networking time and activities.
