Dr Matthew Fay is a General Practitioner who joined Westcliffe Medical Practice in 1999. This has developed into the Affinity Care, a 29-partner super practice based in Bradford providing care for over 64,000 population. Matthew is the Clinical Chair but still does regular GP sessions at The Willows in Queensbury.

The i2i Network is delighted to bring to you this two-part virtual workshop exploring the role of pharmacists in managing people living with chronic heart failure with reduced ejection fraction (HFrEF) in primary care.

In part 1 of the series, our expert speakers; Rocco Hadland and Matthew Fay (see bios below), will provide insight on the treatment and management options for chronic heart failure, whilst in part 2 they will focus on building your knowledge and confidence to start implementing change in your practice.

Part 1

Wednesday 16th June—20:00-21:30

  1. Explain the role of primary care and the unmet need in managing chronic heart failure with reduced ejection fraction (HFrEF)
  2. Understand best practice in the management of chronic HFrEF from a clinical and service delivery perspective
  3. Consider treatment options and the opportunity for the GP pharmacist and primary care to achieve optimal outcomes in chronic HFrEF

Part 2

Wednesday 7th July—20:00-21:30

  1. Sharing of the Bradford heart failure pathway and discussion of the drivers and barriers to successful implementation of pharmacist-led clinics for HFrEF in primary care
  2. Walk though a practical population case study for implementing pharmacist-led review clinics in chronic HFrEF
  3. To provide an implementation route map supporting pharmacists to drive multidisciplinary team service change in chronic HFrEF
