is the Lead Pharmacist for Thrombosis and Anticoagulation at the Royal United Hospital (RUH), Bath where he has helped optimise anticoagulation services, including the setting up of a pharmacist led cancer associated thrombosis clinic. He has previously worked as a rotational clinical pharmacist in secondary care and as a GP medicines optimisation pharmacist for Bristol Clinical Commissioning Group (CCG).

Agenda

Part 1 of the workshop will focus on the acute management of VTE and part 2 will focus on long-term secondary prevention.

Following part 1, you will:

  • Understand the symptoms and risk factors for VTE
  • Understand how VTE is diagnosed
  • Understand the patient factors to consider when choosing anticoagulation for acute management
  • Know how anticoagulation is initiated and how patients should be counselled on their treatment

Following part 2, you will:

  • Understand risk factors for recurrence of VTE
  • Understand when anticoagulation for long-term secondary prevention is appropriate
  • Understand how practice pharmacists can support patients taking long term anticoagulation
  • Know how to apply HAS-BLED in VTE
