What is the Clinical Champions Cardio-Renal-Metabolic (CRM) Programme?

Our CRM Clinical Champions Programme, specifically designed for GP clinical pharmacists already seeking to deepen your clinical competence whilst equipping you with the quality improvement (QI) skills and tools to develop and deliver a CRM service. The programme will be delivered by a faculty of national and regional level consultant and specialist pharmacist experts in the cardiovascular disease (CVD), diabetes, renal and heart failure specialisms and will be underpinned by nationally agreed competency framework.

This programme is ideal for clinical pharmacists who are already running type 2 diabetes mellitus (T2DM), CVD or heart failure clinics and want to expand clinical scope and have the intention and organisational commitment to drive service and QI change within 12 months.

What is unique about the programme?

The programme utilises the 4D® methodology which encompasses four stages, DIAGNOSE, DESIGN, DELIVER and DEMONSTRATE which you will work through with your chosen CRM QI project. The programme offers a structured approach to learning that combines both clinical training and QI-focused workshops.  The outputs of this programme will provide evidence of delivering services and clinical leadership in line with the Advanced Pharmacy Framework (APF) which can support future credentialing activities.

What is the time commitment and structure?

To successfully complete the programme, participants are expected to commit to attending ALL scheduled workshops, both face-to-face and virtual, over six months from February to July 2026. Active participation in leading a CRM service improvement project and completing associated tasks is also required to maximise the learning experience and contribute meaningfully to the programme’s goals.

  • The programme is structured over 6 months (February – July 2026) with a training intervention each month
  • Two whole day face-to-face workshops at the start and the end alongside four 2.5 hour virtual sessions – led by clinical experts
  • Homework will be provided after each session
  • The programme will be supported with implementation resources such as a fishbone diagram, programme GANTT charts, risk register and options appraisal tool
