Mohammed Swaleh

Mohammed Swaleh is a Lead Clinical Pharmacist and Independent Prescriber in Osteoporosis with extensive experience across NHS primary care, Medicines Management Teams, hospital services, and community pharmacy. He specialises in medicines optimisation, clinical leadership, digital transformation, quality improvement, workforce development, and service innovation. He provides strategic leadership across medicines optimisation, independent prescribing, clinical governance, workforce development, education, quality improvement, and digital innovation for Your Family Practices. Mohammed has extensive expertise in long-term condition management, including cardiovascular disease, chronic kidney disease, osteoporosis, heart failure, anticoagulation, and bone health. Throughout his career, he has developed prescribing guidance, clinical pathways, Standard Operating Procedures (SOPs), medicines safety initiatives, and quality improvement programmes, while leading prescribing optimisation projects within NHS Medicines Management Teams to improve patient safety, evidence-based prescribing, clinical governance, and cost-effective care.

A key focus of Mohammed’s work is driving service improvement through innovation and digital transformation by developing practical, data-driven solutions that strengthen medicines optimisation, improve clinical workflows, enhance governance, and increase efficiency. He has introduced the responsible use of Artificial Intelligence (AI) across the pharmacy team to support clinical audits, quality improvement, educational resources, SOP development, business cases, and project work, with all clinical outputs remaining professionally reviewed and aligned with NHS guidance. Mohammed is also passionate about workforce development, mentoring multidisciplinary teams including pharmacists, pharmacy technicians, pharmacy assistants, Independent Prescribers, and university students. He has established successful partnerships with Brighton & Kingston Universities to create structured pharmacy student placement programmes and has delivered education and training to GPs, nurses, physiotherapists, and other primary care professionals, supporting the practice’s ambition to become a recognised teaching practice, developing the future NHS workforce, and creating sustainable additional income streams.
