Poh is an Advanced Pharmacist Practitioner and Independent Prescriber at Chiswick PCN, where she also serves as a dedicated Respiratory Champion, and a Clinical Lead for Primary Care Training at Hounslow Consortium.
With a strong track record in leadership, Poh previously served as Head of PCN Pharmacy in Chiswick, leading her team to become finalists at the national General Practice Awards 2024. She works closely with multidisciplinary colleagues to promote face-to-face multiprofessional educational events across the five PCNs in Hounslow, fostering networking and collaboration amongst healthcare professionals.
Poh is Joint Lead for the Respiratory Diagnostic Hub and a Sub-Investigator on a commercial research study focused on severe asthma, reflecting her commitment to advancing evidence-based practice. She continues to dedicate time to direct clinical work, bringing real-world insight and passion for improving patient outcomes to everything she does.
