Leanna is a PCN Clinical Pharmacist and Independent Prescriber at Wallington PCN, with a strong interest in CVRM and the holistic management of patients with complex long-term conditions. Her work focuses on improving medicines optimisation, supporting earlier intervention and ensuring patients receive coordinated, person-centred care.
She is also a delegate of the 2026 CRM Clinical Champions cohort and will be presenting a poster on her work to strengthen the identification, optimisation and ongoing management of patients with CKD in primary care, with the aim of improving outcomes and reducing the risk of disease progression.
