Rachel Vincent is a Senior Lead Clinical Pharmacist for Integrated Care at The Newcastle upon Tyne Hospitals NHS Foundation Trust, where she has worked since 2016, clinically delivering sessions within the Inner West Primary Care Network in Newcastle, providing strategic and clinical leadership across general practice settings and also to Outer West Primary care network.
A highly experienced clinical pharmacist, Rachel is widely recognised for her passion and expertise in Type 2 diabetes, cardio‑renal‑metabolic health, and diabetes technology. Her work is driven by a commitment to reducing health inequalities, improving access to high‑quality care, and empowering patients to achieve better long‑term health outcomes.
Before joining Newcastle, Rachel held the position of Lead Pharmacist for Infectious Diseases at Leeds Teaching Hospitals, where she developed specialist skills in antimicrobial stewardship, infection management, OPAT MDRTB and HIV and multidisciplinary clinical leadership so an original Yorkshire lass!
Across her career, Rachel has been instrumental in developing innovative clinical pathways, supporting PCN‑level transformation, and advancing the role of pharmacists within integrated care. She continues to champion evidence‑based practice, collaborative working, and the advancement of personalised care for people living with long‑term conditions.
