Diabetes Data Capture Project (DDCP)!

Calling all #GPPharmacists in Devon!

The i2i team are proud to initiate our first implementation project in Devon to showcase the clinical outcomes achieved by the practice pharmacists in Type 2 Diabetes!

We will be working closely with Dr David Strain, (FRCP MD Clinical senior lecturer and honorary consultant at the University of Exeter Medical School, Institute of Biomedical and Clinical Science and Department of Diabetes and Vascular Research) in this project to showcase the impact of practice based pharmacists on reducing clinical inertia in Type 2 Diabetes.

The primary outcomes that the pharmacists will be focussing on over the 12 month data capture project are:

1 – Changes in HbA1c

2 – Changes in cholesterol levels

3 – Number of hospital admissions for hypoglycaemia attacks
