Innovation and Creative Speaker

Julie Creffield is an Innovation Architect for the AI Age, and author of the book You, Me & ChatGPT, with over 30 years’ experience helping professionals navigate disruption, redefine expertise, and lead through change.

Her work focuses on the human side of AI adoption — not just the tools, but the thinking required to use them well. She is known for challenging assumptions about expertise in an era where technology can replicate tasks, but not judgement.

PCN IP Clinical Pharmacist (Specialist Interest in Cholesterol), South Hams PCN

Affreen Mohammed MPharm PGDip IP is a Clinical Pharmacist at South Hams PCN, specialising in cholesterol management and cardiovascular disease prevention. She does CVD and CKD optimisation across the PCN, heads the Community of Practice for Hypertension, and represents primary care pharmacy on the Devon ICB CVD Prevention Group.

Bisola Sonoiki

Independent Prescribing Pharmacist, Co-chair South West Inclusive Pharmacy Practice

Bisola is a results-driven PCN Pharmacist with broad experience across community pharmacy, hospital services, an Integrated Care Board and, more recently, general practice. This varied background has given her a strong understanding of medicines optimisation, patient care and the different challenges faced across healthcare settings.

In her substantive primary care role, she is committed to improving patient outcomes through safe, evidence-based and person-centred care. Bisola was also a delegate on the 2025 i2i CRM Clinical Champions cohort, where she further developed her knowledge, leadership skills and ability to deliver meaningful improvements in cardio-renal-metabolic care.

Lisa Clift

Advanced Clinical Practitioner Pharmacist & Clinical Pharmacist Facilitator, Wrekin PCN

Lisa Clift is an Advanced Clinical Practitioner Pharmacist and Clinical Pharmacist Facilitator at Wrekin PCN. She qualified as a pharmacist in 2011 and as an Independent Prescriber in 2023, following 10 years in community pharmacy.

She has a particular interest in respiratory care and a strong passion for education, mentoring pharmacy technicians and supporting the development of clinical pharmacists. Through her facilitator role with the Shropshire, Telford and Wrekin Training Hub, she also delivers training and strengthens collaboration across community, primary and secondary care.

Clinical Pharmacist at Dawley Medical Practice

Surinder Kumar is a Clinical Pharmacist at Dawley Medical Practice with an interest in medicines optimisation and improving the management of long-term conditions in primary care. His work focuses on supporting evidence-based prescribing and helping patients achieve better outcomes through proactive, person-centred care.

He will be speaking on optimising migraine prevention in practice, sharing practical approaches to identifying suitable patients, reviewing treatment and improving the use of preventive therapies.

Senior Advanced Practice, Clinical Pharmacist and Lead Pharmacist for Wallsend Primary Care Network (PCN)

Mary is a Senior Advanced Practice Clinical Pharmacist and Lead Pharmacist for Wallsend Primary Care Network (PCN), leading the pharmacy team and supporting the delivery of high-quality clinical pharmacy services across general practice. As an Independent Prescriber, Clinical Supervisor, and Designated Prescribing Practitioner (DPP), she is passionate about workforce development, mentoring pharmacists, and advancing clinical practice in primary care.

Janice Headon

Clinical Pharmacist South Molton Medical Centre

Janice has been a qualified pharmacist for 17 years and has held a range of roles across community pharmacy, a CCG medicines optimisation team, now part of the ICB, and most recently general practice as a clinical pharmacist.

She is an Independent Prescriber and has completed her advanced practice portfolio through the Royal Pharmaceutical Society. Her role is broad and varied, spanning process improvement within the practice, direct clinical care and, more recently, the development of the surgery’s clinical research offering.

Clinical Pharmacist Prescriber, Homewell Practice

Emily is an advanced clinical pharmacist with over 20 years of experience across community, hospital and primary care. She has a specialist interest in cardiovascular, renal and metabolic medicine (CVRM) and works nationally in areas such as diabetes technology and lipids optimisation. Emily won the CPC 2021 Award for Excellence in General Practice, and the C&D Award for GP/PCN Pharmacist of the Year 2025.

Dr Paul Downie

Consultant Lipidologist/Chemical Pathologist and Specialist CVD advisor, University Hospitals Bristol NHS Foundation Trust

Dr Downie is a Consultant Lipidologist and Chemical Pathologist at University Hospitals Bristol NHS Foundation Trust and Specialist CVD Advisor to the West of England Health Innovation Network. His interests include lipid optimisation in secondary prevention, lipoprotein(a) and hypertriglyceridaemia. He established the only NGS assay included in the National Genomics Test Directory for investigating genetic causes of severe hypertriglyceridaemia. A former GP partner, he brings a pragmatic understanding of the challenges facing primary care and is passionate about education and supporting colleagues to optimise community-based CVD management.
