Innovation and Creative Speaker

Julie Creffield is an Innovation Architect for the AI Age, and author of the book You, Me & ChatGPT, with over 30 years’ experience helping professionals navigate disruption, redefine expertise, and lead through change.

Her work focuses on the human side of AI adoption — not just the tools, but the thinking required to use them well. She is known for challenging assumptions about expertise in an era where technology can replicate tasks, but not judgement.

Accomplished Associate Director, Lead Clinical Pharmacist, and Advanced Clinical Practitioner

Chaandni Kay Devgon is an accomplished Associate Director of Medicines Management, Lead Pharmacist, Advanced Clinical Practitioner, and Independent Prescriber based at Central Surgery and across local Primary Care Networks (PCNs) in Surrey. She has a specialist interest in cardio-renal and metabolic medicine, with extensive expertise in patient care, clinical pharmacy, and medicines optimisation.

Lead PCN Clinical Pharmacist, Cardio Renal Metabolic Care, Clinical Strategy & Workforce Development

Deviata is a Lead PCN Clinical Pharmacist with a specialist interest in CKD, hypertension, lipid management and the wider cardio-renal-metabolic spectrum. She delivers advanced medicines optimisation for complex patients, including those living with frailty and polypharmacy, and works closely with care homes to improve safety and patient outcomes.

Maha Gad

Primary Care Pharmacist, WellBN PCN, Brighton

Maha is a Primary Care Pharmacist in Brighton with nearly three years’ experience in the role. After completing her pre-registration training, she worked as a relief pharmacist before progressing to Pharmacist Manager, a position she held for eight years before moving into primary care.

She has a particular interest in respiratory care, especially asthma, and completed her Independent Prescribing qualification in this area. She has further developed her expertise through shadowing respiratory consultants and attending educational events, using this learning to strengthen patient consultations and support colleagues.

Lead Clinical Pharmacist and PCN Managing Lead Pharmacist, Your Family Practice

Mohammed Swaleh is a Lead Clinical Pharmacist and Independent Prescriber in Osteoporosis with broad experience across NHS primary care, medicines management, hospital services and community pharmacy. He provides strategic leadership for Your Family Practices across medicines optimisation, prescribing, clinical governance, workforce development, education, quality improvement and digital innovation. His clinical expertise includes cardiovascular disease, chronic kidney disease, osteoporosis, heart failure, anticoagulation and bone health.

Poh Long

Advanced Pharmacist Practitioner and Respiratory Champion, Chiswick PCN, Clinical Lead for Primary Care Training, Hounslow Consortium

Poh is an Advanced Pharmacist Practitioner and Independent Prescriber at Chiswick PCN, where she is Respiratory Champion, and Clinical Lead for Primary Care Training at Hounslow Consortium. Previously Head of PCN Pharmacy, she led the Chiswick team to become finalists in the 2024 General Practice Awards. She now supports education and collaboration across Hounslow’s five PCNs. Poh is also Joint Lead for the Respiratory Diagnostic Hub and a Sub-Investigator on a severe asthma study, while continuing to deliver direct clinical care.

Independent Prescribing Pharmacist, Shakespeare Health Centre, London

Janki is an independent prescribing pharmacist with a particular interest in cardio-renal-metabolic health and delivering holistic, patient-centred care. She previously participated in the i2i QI PODS programme and has since progressed to become a mentor on the CRM Clinical Champions programme, supporting other pharmacists to develop and implement quality improvement projects within their own services.

Niraj Lakhani is a Independent Prescriber and Lead Clinical Pharmacist with a PG Dip in Clinical Pharmacy. He is passionate about delivering high-quality, patient-centred care, clinical governance and quality improvement.

With extensive clinical knowledge and a strong understanding of NHS and patient pathways, Niraj is committed to reducing health inequalities, improving patient and prescribing safety, and optimising the management of long-term conditions. He is also passionate about developing pharmacy professionals through mentorship and expanding the role of pharmacists as integral members of the multidisciplinary team.
